Working in this setting allows you to practice with an interdisciplinary team and greatly impact the lives of patients, as you build one-on-one relationships. We are looking for expert generalists to provide health teaching to patients and caregivers, preventive health screening, referral and coordination of services in the agency and the community, and advocacy for the patient to obtain health-related goals.
